At Falcons Landing we have a wide variety of healthcare services right here on our campus. Our health center provides Five-Star CMS rated nursing care. We also offer short-term inpatient rehabilitation following a hospitalization or surgery, respite stays (upon availability) and out patient rehab services. Licensed nurses and certified nursing assistants are onsite 24 hours a day.
The Johnson Center at Falcons Landing has been recognized again as a Best Nursing Home for Short-Term Rehabilitation in the 2024 U.S. News & World Report. We are proud to have been given the highest distinction from the Centers for Medicare and Medicaid Services - a Five Star High Performance rating for both Short-Term Rehabilitation and Long-Term Care.
The Johnson Center and West Falls Center welcome all members from the local community, no military or government background needed. If you or a loved one need 24-hour care, whether short-term or long-term, Falcons Landing is there for you!

Our community offers two assisted living options: West Falls Center and The Johnson Center.
Our Assisted Living team provides services to support our residents and foster their highest level of independence. A personal plan is created foreach resident and may include such services as medication management and assistance with bathing and dressing, if needed.
Located on Falcons Landing's beautifully landscaped main campus, The Johnson Center, is home to our Skilled Nursing center. Here we offer Short-Term Rehabilitation, Short-Term Respite stays and Long-Term care. Licensed nurses and certified nursing assistants are there 24-hours a day to provide the care and assistance for residents need.
